When Carmen wrote that module she assumed no-one married more than 5 times. As your error says “Undefined offset: 5” , which with numbering starting at zero means a 6th occurrence, I guess there is someone in that family line that married at least 6 times. Is that the case?

The attached file will give you a temporary solution. Meanwhile I have suggested a better alternative than a 5 marriage limit to Carmen.
The file replaces module.php in kiwitrees/modules_v3/fancy_treeview
