    Ah, now I see. Interesting case.

    I would use OPTION 2 and add a NOTE to explain the two married names.

    Or you could use OPTION 1 and similarly add the same note to the third name group (BLUSTEINOWITZ).

    Either are valid. Opt 1 is the more technically correct. Opt 2 is (IMHO) the more visually pleasing and certainly a lot less effort!
